1. Athletic Training Center Requirements
❐ Pre-participation Physical Examination (PPExam)
❐ Complete the Athletics Electronic Medical Record
❐ Provide Your Sickle Cell Trait
❐ Complete a Baseline Concussion Test

1. Athletic Training Center Requirements
❐ Pre-participation Physical Examination (PPExam)
○ Submit the form of your doctor’s choosing to Chris Barry (contact info below)
○ PPExam must be within 6 months of the 1st practice
○ Fall sport Athletes must obtain a PPExam after March 1st
○ Spring sport athletes must obtain a PPExam after April 1st
■ April 1st coincides with non-traditional practices/tryouts.
❐ Complete the Athletics Electronic Medical Record
○ Go to SportsWare Online (SWOL)
■ Click “Join SportsWare” and enter the school ID: MMABUCS.
■ IMPORTANT: you must use your MARITIME EMAIL ADDRESS.
■ Once granted access to the site, complete the 7 Forms, Athlete Information, and Medical History (including all Required and asterisked (*) information).
■ Information will be reviewed, and the status will be manually updated
❐ Provide Your Sickle Cell Trait
○ Contact your doctor or the State your were born in for information regarding newborn sickle cell trait testing
○ Testing locations, such as Quest Diagnostics, offer the blood test for a fee.
○ Note: The NCAA no longer allows waivers for this requirement.

❐ Complete a Baseline Concussion Test
○ Contact Chris Barry for instructions in completing the SWAY concussion test.
2. Compliance Requirements
❐ Title IX Incoming Student Attestation Form (Required by NCAA)
● Go to Accepted Student Section of the MMA Website
● Click “Login to Status Portal”
● Use your Slate Login / Password to log on
● Click “NCAA Title IX Disclosure Form.”
● Login with your MyMaritime password
● You will find the form in eTrieve/Softdocs
● Complete the form
○ Since the form is outside of Slate, your checklist may take a day to update, saying the form has been completed
❐ NCAA Compliance Online Profile
● Go to the NCAA Eligibility Center website
● Create a free NCAA ID profile using your maritime.edu email.
